Home & Community Based Services | Medically Fragile Community Care Program

Programs and Services

For over 30 years, Responsive Caregivers of Hawaii has supported adults with developmental disabilities. Our care services are developed under a strong foundation: to help each individual with special needs to be integrated successfully in the community.

We understand that each individual needs different care plans and assistance, and this is why our care services are personalized to meet each individuals’ interests, physical challenges, and medical requirements.

Under our Home & Community Based Services, we offer a wide range of care programs for individuals with developmental disabilities aged 22 and over. The services include the Adult Day Health Program, Personal Assistance, Respite Care, and Habilitation services.

In 2003, Responsive Caregivers of Hawaii developed a new care program called Medically Fragile Community Care Program (MFCCP). This care program, primarily including case management and care coordination services, is designed for children who are medically fragile and their families.
